Form Builder & Management

Appello's form builder lets you create custom digital forms for safety inspections, toolbox talks, quality checklists, incident reports, and any field documentation. The drag-and-drop builder supports conditional logic, multiple field types, digital signatures, and GPS-stamped submissions — replacing paper forms entirely.

Overview

Form Management — list of 20 active form templates with name, version, published date, and description columns

The Form Management page lists all form templates in your organization. Each template can be:

  • Active — Available for field workers to fill out
  • Draft — In progress, not yet available
  • Archived — No longer active, but historical submissions are preserved

Building a Form

Starting a New Form

  1. Click Add New to open the form builder.
  2. Enter a form name (e.g., "Daily Safety Inspection Checklist").
  3. Start dragging field types onto the canvas.

Conditional Logic

Create forms that adapt based on responses:

  • Show/Hide fields — If "Hazard Found?" = Yes → show "Describe Hazard" and "Photo of Hazard"
  • Required conditionally — If "Working at Heights?" = Yes → "Fall Protection Type" becomes required
  • Skip sections — If "N/A" is selected, skip the entire section

Appello includes a library of industry-standard form templates that you can use as-is or customize:

To use a template:

  1. Browse the template library.
  2. Click Use Template on the desired form.
  3. Customize fields, add your company branding.
  4. Activate the form.

Assigning Forms to Jobs

Forms can be assigned as mandatory for specific jobs:

  1. Open a job detail page.
  2. Navigate to the Forms tab.
  3. Click Assign Form and select the template(s).
  4. When a worker is scheduled to that job, the assigned forms appear in their mobile app's My Forms tab.

Version Control

When you edit an active form:

  • A new version is created.
  • Previous submissions retain the form version they were filled out under.
  • Historical data is preserved even as form templates evolve.

Form Duplication

Duplicate an existing form to create variations:

  1. Open the form you want to copy.
  2. Click Duplicate.
  3. The new form opens in the builder with all fields pre-populated.
  4. Make changes and save with a new name.
